Service Description

This service is for Laboratory delivery only. If you have collected a sample on your own (stool, urine) we will deliver the specimen to the laboratory of your choice. This service requires a lab order from a physician indicating which tests should be collected. After booking your appointment, please email your lab order, insurance card, and ID to Kacey@feathertouchphleb.com We cannot confirm your appointment until we receive your lab order. Price listed is out of pocket. Services are not covered by insurance. Insurance covers laboratory testing and processing fees ONLY. Additional charges may apply for exceeding 30 miles.
